Thank you for your interest in scheduling an appointment with our office. We invite you to schedule your appointment from your secure Personal Health Record (PHR) Portal. We have provided to help page for the portal.

If you are a new patient, great! We look forward to meeting you! Please click the button above to register as a new patient and receive access to your own PHR with our office. Once you have registered, you will receive a verification e-mail with a link back to the PHR. Click that link and you’re off to schedule your appointment.

If we’ve seen you before and you have forgotten your username and/or password, please call our office at 440-571-7100 and we’d be happy to help. Once inside your PHR, you’ll be able to search for and schedule your visit.
